Some important characteristics with two major pandemic viruses

Postoperative infection after elective arthrodesis of the interphalangeal joint is an uncommon complication often necessitating urgent debridement. We present the rare case of a female patient with a history of oral herpetic lesions, who underwent elective arthrodesis of the middle and index fingers for treatment of erosive osteoarthritis and subsequently developed a postoperative herpetic infection at the surgical site.Porcine epidemic diarrhea virus (PEDV) has been recognized as a swine pathogen for 40 years, but until 2013 had not been detected in the Western Hemisphere. From originally causing a relatively mild and sporadic disease, PEDV has been more recently associated with severe outbreaks of diarrheal disease in Asia, and subsequently North America. PEDV shares some important characteristics with two major pandemic viruses (porcine reproductive and respiratory virus; porcine circovirus type 2) of pigs that have high rates of mutation and high host specificity, and appear to have been present in the swine virome for decades prior to emerging to cause severe clinical disease. A unique feature of the PEDV in North America has been the implication of feed as a vehicle for transmission, with particular concerns related to ingredients of porcine origin. The importance of relatively rare events in contributing to both the emergence and transmission of PEDV is discussed in relation to approaches for managing the associated risks.

Although half of dilated cardiomyopathy (DCM) cases have unknown causes, the remaining causes of DCM are extensive. In some cases, DCM starts with myocarditis, which is basically an inflammation of the heart muscle. Myocarditis can present in a variety of ways, from asymptomatic to cardiogenic shock. Myocarditis is associated with a wide range of infections, most commonly viral, bacterial, and parasitic. The main mechanism seems to be immune-mediated damage to the myocardium, which leads to a global or local ventricular dysfunction and DCM. Epstein-Barr virus is a rare cause of myocarditis. We could locate only 15 cases (9 adults and 6 children) reported in the medical literature in English. We report an adult patient who presented with signs and symptoms of heart failure most likely secondary to myocarditis caused by Epstein-Barr virus infection. The diagnosis was made by viral serology and a multidisciplinary approach.
